Social Workers can offer a range of supports and are not limited to any one scope of practice. Tara's provision of supports are inclusive of :
- counselling and therapy
- assistance with navigating goverment systems (e.g. Centrelink, My Aged Care, NDIS)
- relationship building
- Psychoeducation
- Functional capacity assessments
Tara is a registered NDIS provider offering Social Work for NDIA, self-managed and plan managed participants under Capacity Building line item 15_621_0128_1_3. This item number is used for therapy, assessments and report writing. Travel costs are billed at the standard rates in line with the NDIS Pricing Arrangements and Price Limits.
Social work therapeutic support is a specialised service focusing on the psychosocial aspects of participants' lives. It addresses emotional, social, and psychological challenges commonly experienced by individuals with disabilities. Social workers in the NDIS offer therapeutic interventions, counseling, and support to help participants develop coping strategies, increase resilience, and enhance their mental health and overall well-being.
Medicare rebates are offered under a Mental Health Treatment Plan which can be obtained from your GP or Psychiatrist. A gap fee of is charged upon session attendance. Sessions are 50 minute in duration. Please see below for the cancellation policy.
Tara provides Functional Capacity Assessments for Psychocosocial Disabilties for the NDIS.
A comprehensive evaluation, known as a functional capacity assessment, is to gauge an individual's capabilities and restrictions in executing diverse activities of daily living, work-related responsibilities, and other functional tasks. This assessment usually includes the observation, testing, and documentation of the individual's physical, cognitive, and emotional abilities. The duration of these assessments is capped at 14 hours which includes 4 hours face-to-face contact for psychometric testing and evaluation.
Social work services within home care packages encompass a range of activities aimed at addressing the social, emotional, and practical needs of individuals. Tara provides:
Counselling: Providing emotional support, counseling, and guidance to individuals and their families to cope with life changes, health issues, or other challenges.
Advocacy: Social workers advocate on behalf of clients to ensure they receive appropriate and quality care, services, and resources.
Information and Education: Providing information and education on available services, benefits, and resources to help individuals make informed decisions.
Tara works with you and your provider to enhance your overall well-being, dignity and independence within your own home.
Tara can provide Social Work services to NDIS and Home Care Package participants in their home. Home visits are located within the Sunshine Coast and South of Gympie area. Travel costs are charged.
Work Cover counselling is provided to employees who have been injured on the job and are struggling to come to terms with the physical and emotional impact of their injury.
Telehealth counselling can be an effective way to receive therapy services, particularly for individuals who have limited access to traditional in-person therapy due to location, physical disabilities, or other constraints. Telehealth counselling allows individuals to access therapy services from the comfort of their own homes or other convenient locations, using a computer, tablet, or smartphone.
